Care Coordination Service

Care Coordination

The Care Coordination Service delivered from the children's centres was introduced in 2018 as phase 2 of the Integrated Service for Children with Additional Needs (ISCAN)


The service aims to support and empower families whose child receives input from two or more services, to navigate and organise their child's care. The following information is for practitioners who wish to find out more about the service or develop a similar service in their area. If you would like to discuss Care Coordination further, please contact the ISCAN Regional Team Lead, Joanna Box, on joanna.box@wales.nhs.uk.

Families cannot self-refer to Care Coordination; eligible families referred via ISCAN are allocated to a Care Coordinator or Family Support Worker. If you are a parent or carer of a child with a disability or developmental difficulty and feel you need support, please contact one of our Family Liaison Officers who will be able to help:
